All coaches and assistant coaches are required to submit to a fingerprint based background check.  This is part of the "Live Scan" program run by the California Department of Justice and it to intended to identify those individuals that have been convicted of committing crimes against others, and prevent those individuals from participating in our program as a coach or assistant.

The cost of submitting to the test is $15, and it can be done at the San Benito County Office of Education in Hollister as well as at dozens of other locations throughout the State. You are free to select a location convenient to you.  IT IS COMPLETELY CONFIDENTIAL  AND YOUR RECORDS WILL NOT BE SHARED WITH ANYONE!!  Only our League Risk and Management Representative  will be notified if there is an issue, and he/she has signed a confidentiality agreement with the State.

2) COMPLETE AND BRING 3 (THREE) COPIES OF THE FORM. YOU MUST COMPLETE YOUR SECTIONS OF THE FORM AND BRING IT WITH YOU WHEN YOU GO TO GET FINGER PRINTED.
